What is home care?

Home care is medical care provided in a patient’s home. Home health care can include care given by skilled medical professionals, including skilled nursing care, physical therapy, occupational therapy and speech therapy. Home health care can also include skilled, non-medical care, such as medical social services or assistance with daily living from a highly qualified home health aide. Home health providers are committed to quality improvement and are interested in working as partners with hospitals to prevent unnecessary hospital readmissions.

What is considered skilled services?

Skilled Nursing Service:

A skilled nursing service is a service that must be provided by a registered nurse, or a licensed practical (vocational) nurse under the supervision of a registered nurse, to be safe and effective. In determining whether a service requires the skills of a nurse, consider both the complexity of the service, the condition of the patient and accepted standards of medical and nursing practice. Some services may be classified as a skilled nursing service on the basis of complexity alone, e.g., intravenous and intramuscular injections or insertion of catheters, and if reasonable and necessary to the treatment of the patient’s illness or injury, would be covered on that basis.

Skilled Therapy Service:

The service of a physical, speech-language pathologist or occupational therapist is a skilled therapy service if the complexity of the service is such that it can be performed safely and/or effectively only by or under the general supervision of a skilled therapist. To be covered, the skilled services must also be reasonable and necessary to the treatment of the patient’s illness or injury or to the restoration of maintenance of function affected by the patient’s illness or injury. It is necessary to determine whether individual therapy services are skilled and whether, in view of the patient’s overall condition, skilled management of the services provided is needed although many or all of the specific services needed to treat the illness or injury do not require the skills of a therapist.

How long will you be in my home?

Your plan of care will be for approximately 2 months and any changes will be determined by you, your doctor and your care team. 

What does a Home Health Aide do for me?

Your Home Health Aide will assist you with your bathing, dressing, oral care, hygiene, toileting, changing bedding and light housekeeping. They will be in your home usually 2-3 days per week depending on your needs. 

What can I expect from therapy?

You may be receiving physical, occupational and/or speech therapy throughout your plan of care. The therapist that will come to evaluate you will be an Occupational or Physical Therapist and then they will work closely with an Occupational Therapy Assistant or Physical Therapy Assistant that will complete the visits. The Speech Therapist will evaluate and complete the visits all on their own.

Does my doctor know what is going on during the process?

We have excellent relationships with the doctors and nurses and make them aware of your condition as it changes. We will then inform you right away of any order received from your doctor. We work closely with your doctor to ensure your speedy recovery.

How will I know when everyone is coming?

We will provide you with a three-ring binder that has all the information we discussed during you initial meeting with the nurse. We also provide a calendar that our staff will their appointment dates and times on. Also, you can feel free to call the office at any time with questions.

How much are the services going to cost me?

We work hard to ensure that all costs are discussed prior to admission to our company. We will call the insurance and get approve for treatment if needed. We are Medicare approved.

What if I feel I am ready to discharge before you do?

You will be a part of the planning throughout the entire process. You will help develop goals and will know the time frame for meeting those goals. If you met your goals early we will call and discuss this with the doctor you and your HH team to make an appropriate decision.

Will I see the same people all the time?

We try our best to get you with the same nurse, home health aide and therapist. If the caregiver is sick or on vacation you will have the option of seeing another caregiver.

What if I have questions for the nurse?

You will have access to an on-call nurse 24/7 so you can call our office number and it will be forwarded after hours.

What if I have a doctor’s appointment at the same time, I have a scheduled visit?

Although we will work closely to ensure you are aware of your schedule ahead of time, we know that things come up. You can call into our office and request to change your scheduled visit time and the nurse or therapist will call you.

Will someone tell me if the doctor orders new meds?

Yes, we will be in contact with you as soon as there are any changes that your doctor makes to your plan of care.

Do you have someone on call all the time?

You will have access to an on-call nurse 24/7 for emergencies.

Can someone set up my medications?

Although, Medicare does not pay for us to set up your medications we will work closely with your caregiver and doctor to determine what is the best option for you.

How do I get a walker or any other equipment I might need?

We will work with you to determine your needs and then we will contact your doctor’s office to get an order sent to the pharmacy of your choosing. 

Do you have someone that will come clean my house?

We do not provide cleaning or laundry services however we can provide you with community resources that can help.
