Our physician driven team of professionals will provide nursing to assess all medical needs, physical therapy to help restore mobility, occupational therapy to improve the ability to perform everyday activities, and speech language pathology to help with speech, language, voice quality and cognition, and swallowing issues.
We also provide a medical social worker to help a client and their family connect with community resources and home health aides to provide personal care assistance.
Our intake team will assist in determing your financial eligibility; Medicare, PPO plans with Aetna, Humana, Anthem, and United Health Care. Listed below are the services we provide:
PHYSICAL THERAPY
Assists to restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities in patients with injury or disease. They restore, maintain, and promote overall fitness and health.
OCCUPATIONAL THERAPY
Assists to improve their ability to perform tasks in their daily living and working environments. They work with individuals who have conditions that are mentally, physically, developmentally, socially or emotionally disabling.
SPEECH THERAPY
Assesses speech, language, cognitive-communication, and oral/feeding/swallowing skills to identify types of communication problems (articulation; fluency; voice; receptive and expressive language disorders, etc.) and the best way to treat them.
SKILLED NURSING
Conducts care visits, education of patients, family members and/or care givers on disease processes ; self care techniques; and prevention strategies; coordination of care, providing treatment and services with your physician and other health care team member.
STATE TESTED NURSING ASSISTANTS
Assists the patient in the activities of daily living and provide basic routine care; such as assistance in eating, bathing, brushing teeth, medication prompting, changing dressing, checking a patient’s temperature and pulse rate, and helping with artificial limbs or walking aids.
MEDICAL SOCIAL WORKER
Provides individuals, families, and groups with the psychosocial support needed to cope with chronic, acute, or terminal illnesses. Services include advising family care givers, providing patient education and counseling, and making referrals for other community services.
